Confidentiality Agreement

Hello Again events are designed to provide a safe, loving, and supportive environment for parents to share and move towards healing. To protect our environment, all participants are asked to read, agree and honor the confidentiality agreement. I understand that by volunteering with Hello Again, I may have access to confidential information. I further understand that the information whether electronic, written, or verbal, however, communicated, documented or stored, is strictly confidential. At no time shall I disseminate or cause to have disseminated any information shared, obtained, generated, or in any way related to my participation with Hello Again.

Media Release

By volunteering with Hello Again, my photograph(s) may be taken and my opinion of Hello Again and other comments, will, from time-to-time, be requested. I understand that these photographs and comments maybe used, without further notice, by Hello Again for programing, and educational or marketing purposes and I will not receive any payment from Hello Again for the use of these images or words. As the participant, by signing below, I indicate that I have been given an opportunity to ask questions about this consent and release agreement and I understand and fully agree with the terms.
